Pay range:  $35.20 - $41.15 /hour based on years of experience

Centurion is proud to be the provider of comprehensive services to the Maryland Department of Public Safety and Correctional Services.   We are currently seeking a part-time Discharge Planner Registered Nurse (RN) to join our team at the Home Detention Unit located in Baltimore, Maryland. 

The Discharge Planner RN completes discharge planning for patients referred to the multidisciplinary team based on the determination that community resources will be required at the time of release.  This person will accomplish timely and appropriate discharge plans, act as a liaison between correctional healthcare teams and community agencies.  This person will also collaborate and coordinate all parties regarding patient discharge planning.

Qualifications

• Active RN license to practice in Maryland
• Minimum of one year of experience as a Discharge Planner for a correctional or acute care facility
• Experience working in corrections preferred
• Must be appropriately and actively certified in Cardio-Pulmonary Resuscitation (CPR)/ BLS
• Ability to obtain a security clearance, to include drug screen and criminal background check

Available shift: Part-Time Day Shift (2 8-hour shifts per week)
