MHM Services, Inc. d/b/a Centurion Group, Inc. (“Centurion”) and its subsidiaries (collectively, the “Company”) is pleased to offer this Employee Referral Program as detailed below. If your referral is hired and completes 90 days of continuous employment with the Company, you may be eligible to receive the referral bonus. See the complete rules and details below.

EXCLUSIONS:

Recruiters, hiring managers, or supervisors that have hiring authority over the referred candidate, including Program Managers, Regional Vice Presidents, and Vice Presidents of Operations;
Immediate family members of recruiters, hiring managers, and supervisors who have hiring authority over the referred candidate, including Program Managers, Regional Vice Presidents, and Vice Presidents of Operations

REFERRAL BONUS AMOUNTS:

Full-Time Licensed (Position Provider, Clinician, Nurse) $2,500

Part-Time Licensed Position (Provider, Clinician, Nurse) $1,250

Full-Time Psychologist (Psychologist) $5,000

Critical Needs Positions $TBD

HIRES INELIGIBLE FOR REFERRAL BONUSES:

Referral bonuses will not be awarded for the following referrals:

Incumbents employed by predecessor contractor at the time of contract takeover;
PRN employees unless they are full-time or part-time employees upon completing 90 days of continuous employment;
Locum tenens or agency staff who have worked pursuant to a contract with a Company Entity in the last 12 months;
Former employees who have been employed by a Company Entity in the last 12 months.

PROCESS:

Eligible Employee referring an eligible candidate for a position with the Company must submit their official employee referral online at Centurion.EmployeeReferrals.com.
Referred candidate must accurately identify the referring employee when completing their online employment application. If this information is not listed accurately on the employment application, the bonus will not be awarded to the referring employee.
Assigned recruiter confirms referring employee’s information when interviewing the referred candidate.
The online employee referral submission is valid for 90 days. If the referred candidate is not hired within 90 days of the online employee referral submission, the referral is invalid.

BONUS PAYMENT:

Referral bonuses are payable for qualified referrals that result in a full-time or part-time hire following the referred employee’s completion of 90 days of continuous employment. If the referred employee does not remain employed for 90 continuous days from the date of hire, no referral bonus will be paid. Referral bonuses are not paid on a pro rata basis.
Once the referred employee has completed 90 days of continuous employment, the referral bonus payment will be issued to the referring employee along with the referring employee’s regular wages on the next applicable pay date. The monies are taxable and subject to the same withholdings as regular wages. The referring employee must be employed by a Company entity at the time of the referred employee's completion of 90 days of continuous employment to receive the bonus payment.
The referring employee must have the referred candidate’s approval prior to submitting the employee referral online. In cases where multiple employees refer the same candidate, the applicable Program Manager or Vice President of Operations will decide which referral is eligible; if all referrals of the same candidate are deemed eligible by the applicable Program Manager, the bonus amount will be split evenly between the referring employees.

ACKNOWLEDGMENT:

In order to be eligible to receive a referral bonus, Eligible Employees must sign these terms of service, acknowledging that they have read, understood, and agree to the terms set forth in this Policy.
